The City of Kingston says 33 new transitional housing units at 315 Queen Mary Rd. are expected to open in August as construction on the shared care model nears completion.
The city purchased the former Extendicare building in 2024 to create an integrated facility combining primary health care, transitional housing and community programming aimed at addressing the social determinants of health.
The shared care model includes three components: transitional supportive housing, a primary health-care clinic and community programming space.
The primary health-care clinic, the Midtown Kingston Health Home operated by Kingston Community Health Centres, has relocated to 315 Queen Mary Rd.
Home Base Housing will operate the transitional housing program for adults aged 55 and older with varying health-care needs. The program will provide 24-hour on-site staffing and goal-based case management to help residents transition to more stable housing.
Community programming will begin later this year with a vocational laundry program operated by Lionhearts. Scheduled to open in October, the program will provide participants with practical job training and employment skills. Additional community programming is expected to begin in early 2027.
On July 7, Kingston city council approved plans for a wellness and health centre within the facility’s community space. The centre will complement existing health services by offering respiratory rehabilitation, occupational therapy, physiotherapy, hydrotherapy and other wellness supports.
The city is also accepting applications for a Community Liaison Committee to help maintain communication between partner agencies and area residents. Applications are open through Aug. 9 on the city’s Get Involved Kingston website.
